1 — Personal information

Full legal name, Social Security number last four digits, email, phone, street address, city, state, and ZIP code.

2 — Tax information

Filing status, spouse and dependent information, prior-year details, income sources, deductions, credits, estimated payments, and other facts needed to prepare the return.

3 — Mileage

Review business mileage, vehicle details, parking, and tolls. Personal commuting mileage is not deductible. The form currently references the applicable mileage calculation shown in the intake.

4 — Documents & electronic signature

Upload supporting documents, review the completed information, enter the date, and provide the required electronic signature before secure submission.

Tax filing checklist & mobile access

Clients can start or update returns, check return status, upload supporting documentation, use remote signature capture, and communicate with the preparer.

Self-employment — client & business

Client name, phone, email, SSN last four, business name, industry, EIN, start date, and cash or accrual accounting method.

Self-employment — income & expenses

Gross and other income, returns, cost of goods sold, advertising, vehicle, commissions, depreciation, insurance, interest, professional fees, office expenses, rent, repairs, supplies, taxes, licenses, travel, meals, utilities, wages, and custom expense lines.

Home office & vehicle

Home and office square footage, business-use percentage, mortgage or rent, utilities, insurance, repairs, depreciation, business miles, parking, and tolls.

Employees, retirement & health

Employee benefits, contractor labor, SEP-IRA or Solo 401(k) contributions, health insurance premiums, and related tax-planning information.
